Appointments

I see patients in the office with a schedule that is variable ranging from 10AM to 6PM Tuesday through Friday. Monday is my regular surgery day and I am not usually able to have office appointments on Monday.

IIf you are a new patient we would like you to arrive 15 or 20 minutes before your appointment time to complete paperwork.

If you like, the necessary forms can be printed from the tab "Patient Information Forms" and brought with you to your appointment.

If you would like to make an appointment please go to the "Contact Us Page". It is quite helpful to us if you give us very accurate information about why you need to see the doctor.

We want you to bring your insurance card, a photo ID and a list ( or the bottles) of medications that you are taking.

If you fail to cancel or keep an appointment it will be necessary to make a $25.00 deposit for your next appointment. The $25.00 fee will be waived if the appointment is kept.
